How to Order Birth Certificates

Simple Steps to Order

The first step in ordering a certified birth certificate is to click on the map above and read the specific instructions for your state. From that page you will be instructed to click on the order form and fill it out from your browser window.

Note: Because we need your signature...our order form is not sent to us over the internet. It must be faxed or mailed to us.

Print the order form and sign it. Make sure you have a valid daytime phone number listed and valid E-mail address if you have one. Tell us when you need your request by, and any special instructions you might have. Fax it along with a copy of your drivers license or state issued id card (if needed) to 916-745-4880. Your request can be mailed or sent by air courier to the address below. Call us to determine the total cost if paying by check or money order.

You're Done!
In most cases, orders are processed and arrive as expected. Please contact us if your certificate does not arrive by the posted process time. Notify us at least two business days before your scheduled departure if your certificate is needed for a trip and has not arrived and we will contact the issuing agency.
